Med Card

Med Card is a state-wide initiative to prevent medication errors in patients.

The Med Card is a way for you to make a list of your medications and keep it with you at all times. This single step is the most important thing you can do to help prevent medication errors.

The Med Card comes in several formats for your convenience. (Choose the PDF version if you have any formatting issues with the Word version)
You may wish to save a copy to your computer in addition to keeping a print-out.
